G11 Year End Check Out Form

Dear Gr. 11 students,

Please come to the HS Office to get your Year End Check Out Form. You are supposed to get the books, padlock and any other equipment back to the teacher and ask them to sign on the form. Please return this form to the HS Office by 3:30pm on Tuesday, 13^th June.
